In-home Care for Acute Issues
Our skilled nurses will provide care for a temporary crisis in your home.
Continuous care provided at the home during a short-term, temporary crisis that requires skilled nursing.
Examples of continuous care situations: unrelieved pain, severe nausea and vomiting, or severe shortness of breath.
Requires a minimum of 8 hours of care during a 24-hour day; at least half of that care is provided by a nurse and the other half is provided by a nursing aide or team member.
Care must be approved by the hospice interdisciplinary team and is considered to be short-term care, re-evaluated every 24 hours.
Patients in hospice care will often present a temporary acute crisis that requires more close monitoring and care. Our skilled nurses can go out to your home to provide continuous home care for the uncomfortable or troublesome symptoms caused by these situations. Our team of nurses will give families peace of mind knowing that their loved one is being carefuly monitored and treated.
